How to fill out health assessment???

Ok, please don't think I'm stupid but we just received an email telling us we need to complete our health assessment forms for our upcoming cruise departing Monday from Galveston. Problem is we can't get to the appropriate place to complete it.  We completed our vaccine requirements immediately after booking.  What other information is required as part of the "health assessment" and how would I get to it.  I am not going to download verify.  Can anyone share some advice???

Mine just showed up in my cruise manager today for a cruise Sunday. The carnival web site is very….wonky.

Go to your cruise manager and answer the questions.  Nothing to upload or download

Thanks Elaine but the health assessment form does not appear anywhere in my cruise manager.  Never had this issue in the past. Frustrating.

Finally!!!  The health assessment form just became available on my booking.  Thank you everyone for your help.  Appreciate it!
