Might be hidden some place or I've just not noticed it. Usually the form comes with your documents though we didn't get one for our last cruise. It's pretty short and easy to fill out at check-in. They're going to want to see the card you're charging against anyhow and I think they run it through a machine to see if it's still a good account. You can fill it out ahead of time and simply hand it over when you check in. They just need a signature to process the charges to your card.
