Are outlets standard, or is an adapter needed

There will be both standard US outlets as well as Euro (two prong) outlets. As the others have said, they are typically at the vanity and nowhere near the bed so you will want an extension cord. I don't recall them being three-prong, so you'd want a two-prong extension. I often bring Euro adapters anyway so I can use both sets of plugs. Note that on many ships the plugs are "recessed" so large "wall warts" may not plug into them - I bring a few of those "grounding adapters" (the little plastic two-to-three prong adapters you can buy at the grocery store or hardware store) because they are just the right size to bring the plug flush with the wall. A power strip or extension cord also comes in handy in this case.

 

I had a friend on a previous cruise with a c-pap, and the line actually provided an extension for them, so this may be possible in a pinch as well.
