Documentation for nephew?

We would like to take our 14 yo nephew with us to keep our DS15 company. He has a passport but what else would we need to show we have permission from his parents? Does each cruiseline have their own rules or is there a gov site I can check? TIA

Get the parents to complete and notarize a permission to travel letter. You will also need a medical treatment authorization form completed.
