Post Cruise Tours/Wheelchair

We will be on a cruise next year with someone who is in a wheelchair. We will be disembarking in Hamburg, Germany and would like to spent a week or so seeing some other parts of Europe. I am wondering if anyone can suggest (or provide feedback) a European tour company that specializes with customers in wheelchairs and/or has true accessibility. Often we have been on excursions where they have told us they were prepared for a wheelchair and they really weren't, or at least not as completely as we would have liked.

Can the person in the wheelchair transfer to a standard car? There are some tours that use a mercedes or such. They are expensive but if you can't transfer to the car of little assistance.

Yes, sorry, details would help. She can transfer to a car, and can walk a few steps with assistance, but no stairs or steps (like into a bus).
