does everyone have to fill out the medical documents and get them signed by doc. or only if you have a medical issue???

Only if you or a person traveling with you has a medical condition, that the medical staff needs to be aware of, such as cardiac issues, pregnancy complications, COPD etc.Then the only person that the form is to be required to be filled out for is the one that has the issue, not the entire travel party.

Which form are you talking about, something with specific issues or the one in which they ask if anyone in your party has been sick recently.
