imluvncruzn Posted April 20, 2011 #31 Share Posted April 20, 2011 This just happened to me. I forgot that I had a refund check for $38 from our cruise last January. By the time I finally discovered the check, it had expired so I just thought it was too late. However, I received a letter in the mail from Carnival telling me that the check had expired and that they would like to reissue the check. I had to fill out this simple form and FAX it.
