Health Form

Thanks Cruisenfever I also copied it as it is so much easier that trying to fill it out when you are in line.:)

 

Me too thanks so much, we can get it done before we get in line. :)

It took me less than 1 minute to fill out while the agent was completing her paperwork. I handed her the forms and she gave me the seapass:)

 

I agree. Took about a nanosecond.

 

I like to be prepared, but this is sort of ridiculous.

Just be aware that this is not "the" Health Form that RCI distributes prior to check in. BUT, it is pretty darn close and has been accepted by the check-in agents.

 

We have friends that cruise out of SJU and the last time they checked in, she was given one form and her husband got a different form handed out by the same rep and taken from the sameastack of blank forms that she had in her hand.:eek:

 

All ports may use different forms..........but they all ask the same questions.

I can only speak from my limited personal experience:

 

When I boarded in Port Canaveral, the agent at the desk gave me the form to fill out for my whole family, and it took an extra couple minutes at check-in. Not a big deal.

 

When I boarded in Miami, they left stacks of the papers lying about without any instructions -- we saw other people taking them and figured we were supposed to do the same thing. No pens were provided, and since we were all standing in line we had no place to write; thus, people were sharing pens around and trying to write up against the wall or other people's backs. With no instruction and no cruise personnel nearby, we didn't know if we'd need this to enter the terminal-proper or whether we would need it at the ticket counter. Miami's terminal is a very poorly run operation.
