Disabled Parking Checklist / Required Documents ____ State Issued: Driver's License/I.D. Card/Passport ____ Original Parking Sales Receipt/Parking Ticket Stub (No Copies) ____ Authentic Disability Placard/Permanent Disability License Plate ____ Proof of Sailing (Sailing Card or Original Purchased Ticket Issued from the Cruise Line) ____ Letter/Card from DMV-(Dept. of Motor Vehicles) signifying who the Disability Placard is Assigned to (NOTE:applies to states that do not have a Driver's License # on placard!)
