Puerto Rico Marriage License question

To any recent Puerto Rico brides I have a couple questions on the documents needed for the marriage license. Specifically on the medical certification and affidavit.

 

Did you get some kind of form letter for your doctor to sign (from your wedding planner) or ?

 

On the affidavit, did it have to be certified by your local county clerk, or just notarized by anyone? Similarly did your wedding planner provide or?

The medical certificate requirments are below:

 

You must provide medical certification that shows compliance with tests required for marriage in your place of residence. Be aware that this certification is only valid for 10 days, so you must get married within this 10 day time period.

 

Affidavit requirements:

 

Non-residents need an affidavit stating that you are not a resident of Puerto Rico and that the sole purpose of your visit is to get married. Non-US citizens must also include that you will not be staying in Puerto Rico longer than permitted. Affidavits written in the United States must be accompanied by certification of your state's county clerk. For affidavits written in Republics or Sovereign Countries, the Apostille seal of the Hague Treaty of 1961 is required.
