Permission to reprint photos?

Last year i was given a website to print a form to allow me to take a picturei purchased onboard the ship and make a copy at a local store. Does anyone know what this website is? I would like to make a couple copies of a picture i bought but i know i need this slip so the store will make the copies for me.

I thought you had to buy the pictures on CD in order to be granted a copyright release. No? You can get a copyright release when you buy a print?

 

Last February I bought the CD and a copyright release was included on the CD. It's a form letter and not personalized whatsoever.

You just fill in the info, and then print the release form....no waiting.
