The beds for third and fourth passengers in a cabin are in the wall. At night, your room steward will pull down (or ask for that service if taking an afternoon nap). There's a ladder to climb up. If anyone is a go-to-sleep-with-the-tv-on, they may have trouble seeing the tv.

 

We had an inside cabin, but this photo should give you some idea of the setup (BTW the girl does not come with the cabin):
