You need to be vigilant, as in most cities, but it's pretty safe if you do. The big bus tours like Gray Line or even vans like Viatur spend a lot of time on pick-up and drop-off to various hotels. If you do hire a guide, I'd use him/her to get to off-the-path gems you might not find on your own, and DIY the major sights. Also remember that you need a clear day, not always guaranteed, for good views from the heights (the Cristo, Sugarloaf, any of the vista points).
