Hi has anyone ever added an extra guest to their cabin after all payment was complete ??

Yes, it's possible. The stateroom has to be able to accommodate the extra guest, and the muster station has to have room.

We are sailing in June and just added another room of 2 people. When I was chatting with the rep they said the ship had maxed out its 3 person rooms. So it's possible that your ship could have hot quota. Hopefully it hasn't and your room can accommodate a third!

IF your cabin can accommodate a 3rd person, and IF the "muster station" for your cabin isn't full, it may be allowed. It's always easier to book everyone, and cancel someone if they can't come.
