Travel insurance was bought (from you of course!) for our April 2020 cruise and after it was cancelled we rebooked for April 2021, the insurance planned moved to that trip. We are seriously thinking of not cruising again until 2022 so my question is can you get a refund on the policy or does is stay as a credit until you cruise again.

Specifically for you tlc361, your policy can be switched to the new dates in 2022.

 

For everyone else, most policies will let you hold its value in one way or another, those the rules vary by company. If you are unsure, ask whoever you bought your policy from.
