What medical questions are asked via app pre-boarding?

I went through the process a couple of months ago but my memory is hazy on the details.

Before we board, 1200 the previous day, we receive a reminder to open the app and answer some "health and safety" questions.

 

What are those questions?

 

(I've tried several searches here and cannot find anything...)

Bit off topic but what documents are required at port check in. Is it just express pass, passport and covid vaccine passes?

I think what's required are express pass, passport, covid vaccination certificates showing vaccination dates, and results of (negative) covid test taken within 2 days prior to boarding date.

The ArriveCAN is a separate issue. They have their own app.

Oops, I think I misunderstood your question. The questions displayed above will show in the Celebrity app 24 hours before boarding.
