Online Health Form - Help

I am in New York right now and we have to fill out the online health form prior to our boarding on Friday.

 

How do I do that online for my under 18 year old daughter ?

You should be able to complete the form for everyone on the same booking reference.

I have two booking numbers. One for a cabin for myself and my wife and another booking number for another cabin with my 2 daughters.

 

I can only will out the form for my 20 year old daughter. For the other one they told me to contact guest 1?

OP's problem may be that neither guest on that booking is 21 or over ?

Yes. Second cabin with a 22 and 17 year old person.

Isn't guest 1 the 22 year old?

The Main Guest or whatever they call it has rights to manage the booking overall including for the other guest(s). If you're logged in as Guest 1 you should be able to do whatever is required (I haven't done the health plan yet but I was able to review and approve the contract and COVID notification with and for my Guest #2 when I was logged in as me. AS far as I knowshe hasn't logged into the Personalizer yet.

I have no chance to fill out the form for my youngest daughter.  Hope we will not get any trouble boarding in on Friday?

They will just go through it all when you board, like they do for people who don't bother to supply passport details or a photo online.  It holds up the checking in process but if it's only a few people, they can manage.
