Form for Grandparents

Does anyone have a generic consent form that gives permission for a child to travel with their grandparents. I'd also like it give consent for medical treatment if needed.

I used a more detailed one the last time but for some reason I can't find it on my computer.

 

You can use this as a base. I added things like "Past Medical History", insurance information, medicines that the gparents can give to the children with dosages (things like dosaging for Motrin/Tylenol/Benedryl). You can make it as detailed as you like. And don't forget to get their insurance cards before you leave
