Here's a link to the actual release. You can print it from here. Seauonboard (a CC member) put it into a PDF file to share. YOu can generally make them at the self serve Kiosk, wothout much hassle. I have only been asked one time for a release and that was a Safeway (grocery store) just took my photos and went to Walmart.
