It will only reimburse you for actual cash spent. However, some will cover the cost to have those miles put back into your FF account. This is from TravelSafe: "Trip Cancellation: Benefits will be paid, up to the Maximum Benefit Amount shown in the Schedule of Benefits, to cover You for the unused non-refundable prepaid expenses for Travel Arrangements, including up to $150 for the cost of airline imposed fees to rebank frequent flyer miles for air flights to join Your Trip when You are prevented from taking Your Trip due to . . . To that I will add that to obtain that "re-banking" benefit, you may have to add the re-banking fee to the "cost" of the trip you are insuring even if you don't incur the cost up-front.
