I'm not sure which card I will want to use for my onboard spending - should I just choose cash for now while checking in and I can change it at pier to the credit card?

Choose cash or any card, and you can easily change it at check-in.

Or choose a card now and it is easily changed up to boarding.

Also, I added boyfriend's middle initial that's on passport to name when checking in online, , but it doesn't show up with middle initial on set sail pass. Passport and drivers license match though. Will this be an issue?

The cruise line only cares about first and last name matching the ID documents.

I called Royal Caribbean about this very thing last week. They told me the Set Sail Pass won't show any middle initial. As long as you registered it just as it shows on the passport you'll be fine.
