We, (parents names), consent to allow our child, (child's name), to travel with his grandparents, (our names) aboard the Carnival Triumph departing on October 10, 2015, and returning on October 15, 2015. We then listed our names, date of births and our passport numbers. Below that was a place for the parents to sign and a spot for a notary to witness. If anyone has an example this would help .Thank you very much When I went on my first cruise with my first kids after divorce, he just signed a notarized document with all cruise and port info saying that they could go. Rare lepidoptera Posted September 22, 2016 #7 Share Posted September 22, 2016 Our parents just wrote out a paper stating they were letting the other parent travel with us out of the country with out basic itinary of where and when we would be going and giving consent for medical treatment. They had the note notarized. They started doing this after the first time my Dad took us to Canada after they divorced and we spent probably about 6 or 7 hours at the border while they tried to contact our mom to make sure that we weren't being kidnapped. But after that we were only ever asked for the permission note two times that I remember and we went to Canada at least 3 or 4 times a year.
