They make you fill out the health questionnaire between cruises. So if you answer yes to any of the questions you would be evaluated by the medical staff before being allowed to reboard the ship.

We have done 3 B2Bs on RCL, 3 on HAL and 12 on X and have never had to fill out a health questionnaire on the second leg.

I had to fill in a health questionnaire recently on RCCL whilst doing a B2B as I did in October also! I assume each cruise line has their own set of rules!
