There is no need to use a form at all.

Just call the 800 number and they will do it for you and send you an e mail saying what the new price is or saying what OBC you have received.

It's directly on their home page, under "Customer Service", the link says right on it "Lowest Price Guarantee Claim Form". Not sure how it could be any EASIER to find.

Yeah ! Congrats on the price drop. I just checked and I have one also... I just called customer service and the took care of it.
