When visiting Cuba, what desination

if we are going on an independent tour with an accredited guide, which category do we check?

You would check # 2.  Third party tour.  If you check # 4 you will be asked to redo the form if you do not bring the required license with you.

as motioned before, 2 counts for self scheduled tours. And if you pre booked them in the US, they should be fine.

My friends and I have gotten different answers to this question. Can I wait to get to the pier to answer this? Can visas be charged?

The visa is for Cuba, whereas the affidavit is for the US. So the visa does not have any requirements for how you visit. Iirc, the info on the visa is just your name, birth date, and passport number. 

 

The affidavit is for the US treasury ( I think) making sure that you meet US laws as you visit. It’s filled out prior to leaving, and you keep a copy for a few years. Cuba does not want a copy, nor do they care if you follow that.
