When visiting Cuba, what desination

The visa is for Cuba, whereas the affidavit is for the US. So the visa does not have any requirements for how you visit. Iirc, the info on the visa is just your name, birth date, and passport number. 

 

The affidavit is for the US treasury ( I think) making sure that you meet US laws as you visit. It’s filled out prior to leaving, and you keep a copy for a few years. Cuba does not want a copy, nor do they care if you follow that.
