I think people like the idea of knowing all of their costs upfront, even if it means paying more.

I generally break even on the drink package...sometimes a little ahead, sometimes a little behind, generally $30-$40 in one direction or the other

 

For me, it's the convenience of drinking what I want, when I want, where I want.  I'm not trying to scam the cruise line and drink twice as much as I paid for.
