I would start with the companies that your cruise line recommends.  Normally, they will be able to accomodate any sort of cruise scenario.

They both rent worldwide.    You didn't say what cruise line you will be on but some of them have a limited number of wheelchairs for use by passengers during the cruise.   Check your cruise line's website and search for 'wheelchair'.

If needing only a manual chair, it may be easier to bring from home. If it is a collapsible, manual chair, it should fit in many/most taxis.

it would fly for free on flights from/to the US and not count as baggage/against any baggage allowance.

I borrowed a manual chair from a church where our sewing guild meets! 
 

I’ve also rented them from local medical equipment stores.  
 

Check out Sage Tours, if I recall correctly.  He is in a manual chair and has lots of goodies on his site about accessibility.
