It varies.  On our most recent Regent cruise on August, it was in the breakfast area for all at the hotel.

No doubt A function of how may are boarding the cruise at that port.

It occurs to me that it is also typically a function of the US versus overseas.  Few US hotels (ironically, especially the luxury brands) routinely include breakfast hence the dedicated room or the vouchers mentioned.  Most international hotels do include breakfast so no special arrangements needed (you just provide your room number).  This is consistent with our Regent hotel experiences, both domestically and internationally.

For our March Splendor cruise, the assigned hotel in Miami was the AKA Brickell. The buffet breakfast provided in a ball room was very underwhelming. Had we known, we would have gladly paid in the restaurant ourselves.

This is what I was wondering.  The buffet breakfast at a ballroom/conference room usually is not very good.  We are assigned the Intercontinental Hotel in Miami.  We may go for breakfast somewhere else.

In Rome, breakfast was in a vast ballroom and not great, but was edible and filled a hole till we boarded the ship.

This was completely different to the hotel in LA. There we received a voucher and ate in the restaurant, which was very good.

The difference, approx 400 passengers in Rome and about 20 in LA.
