You should follow the direction provided by NCL for your specific cruise. Don't read stuff from the internet. Rules are constantly changing. Failure to abide by NCL's protocols at the time of sailing 5 months from now (not something you read on the internet) will result in denial of boarding.

All the Bermuda covid rules and forms were ended on November 13. No vaccination, testing, required and no forms need to be filed.
