Pre-pandemic I remember it being offered by cruise lines and also by an independent tour vendor.  (likely the same tour company the cruise lines use).  Also remember being told that Corona films beer commercials there.  Here's a link to the local tour operator:  https://bernardstours.com/tours/pinel-paradise-excursion

Pinel Island is actually really beautiful. I took a private excursion that used it as a restroom break and a souvenir stop, after snorkeling off Tintamarre. Both are really nice. In fact, my whole family has some really nice memories of that experience.
