Should I include cost of tours?

I wonder if I should include the cost of tours in the total price paid when buying an insurance? I'm sailing with Carnival, and already booked and paid for tours, but I paid for them separately from booking.

Are the tours refundable if you cancel? Is there any kind of requirement in your policy that requires you to insure all trip costs?
