whats the shelter from? wind ? rain? spray?

The lower decks on the QM2 (4-5-6) have what they called "in-hull" balconies. The balcony is sheltered with a large opening cut in the hull. You can stand in the balcony and look out over the ocean. There is very little wind or rain. The disadvantages are you cannot see the ocean while seated and you cannot sun yourself.

The upper decks, (8+) have open balconies. The advantages are that you get more light, can sun yourself in good weather and have an open, breezy feel to them. The disadvantages are that during a crossing the wind can be somewhat strong make the balcony unusable and that you are further from the ocean and less likely to see sea life.

The upper decks also feel more of the ship movements than the lower decks. During a crossing, some people prefer to be lower in the ship.
