I'm not aware of a form I sent my request via snail mail to "Revenue Support Dept." requesting the transfer to your TA including the Booking#, etc. My TA gave me a fax # but I don't have it with me, sorry. Send an email from your email address to revenuesupportdepartment@princesscruises.com and include the following information: Your Booking #: Passenger names as they appear on your cruise reservation My travel agency phone #: My agency address: My agent's name: Please copy me (AGENT) on the email so that I know that you have sent it. Princess will contact me to verify the transfer and then I will be back in touch with you. I've always been able to find it before but I can't seem to find it now. I have never used or seen any form for Princess. Yesterday I just transferred 3 cruises to my TA by calling Princess who made a note on my bookings of who they were going to. The TA then called Princess and took them over.
