Here’s a link to their request form & they’ll gladly accept it on any version of the form with any expiration date.  They created the form to fully explain the procedure because of receiving too many incomplete requests requiring extra time to follow-up.  Or you can just provide all of the necessary information by fax or by mailing it to them.

 

Whenever I called requesting a form with a later date they just said to use any version of the form because it hadn’t been recently updated.

I have sent in my info without the form multiple times. Just be sure you have all the required information in your request and it works fine. (I have also sent in my info with the form which, of course, works fine as well.)

I also have never used the form.  Just put the required information in a letter and then attached the proof of ownership.

I've never used a form.  Just fax proof of ownership along with Name, Booking Number, Ship, and date of cruise.  I use "Zero Fax" as it is quick,  easy and cost nothing

For those who are fully aware of the procedure & submit a complete request the form is not necessary.  

 

Princess created the form for people who are unsure of the process & then submit an incomplete request.  That requires extra time to follow up to get all of the required information which slows down the process for everyone.

 

Questions are regularly asked about how to get shareholder OBC so obviously not everyone is knowledgeable about the procedure and for those inexperienced people the form is useful to ensure submitting a complete request.

Sent ours in a week ago via fax with no form, just details on the sheet. It was in our personalizer by evening! They are fast! And I think them for that.
