Info Card to attach to wheelchair for flight

Does anyone have an example of the info sheet they attached to their power wheelchair or scooter for use by the airline's ground crew? All the advice sites say to attach one, but what specifically needs to be on it? I'd love to see what others have used. This will be our first flight with the power chair in 10 years, and the first international flight.

The form for Air Canada is very similar to the one for United.   Different format but basically the same questions.   Air Canada suggest having one copy for them and one copy for the scooter owner.
