My husband's former wife invited my daughter to join them with her step-sister and former wife's new husband on a cruise.  I had to provide a permission letter that was notarized that not only granted permission for my daughter to be taken out of the country but also to make decisions about medical treatment should an emergency occur.  She traveled with this letter and her birth certificate as she was only 14 years old at the time.
