These are offered in every port as well as Regent included and Regent Choice excursions. IMO, you can get a private or small group tour by booking it yourself at much less cost than booking a car from Regent. Regent included excursions generally have at least one or more bus loads of passengers (or boat loads - depending upon the excursion) while Regent Choice excursions (excursions with a cost) are typically one bus load. To add to what TC said, you'll need a car or van for a truly 'private' experience, and you'll pay for that luxury through Regent. Last time we went to Rome, we took the train in and we had a designated rail car, but it was still a Roman mess getting everyone herded together. Probably not as bad as the train from St Petersburg to Moscow, but still a mess. Most of the tours I've taken there have been in the 20-30 person range, with some being as small as 6-8 (usually the Regent Choice, i.e. 'paid' excursions).
