I have the form on my laptop but can't figure out how to attach it here. I think you get it through the Carnival site not Princess. My TA sent it to me. It would be helpful if we could get a link here since this is a question many of us forget how we did it until we need to to it again.

The benefit is applicable on sailings through July 31, 2016 aboard the brands listed below. Certain restrictions apply. Applications to receive these benefits should be made at least

two weeks prior to cruise departure date.

This benefit is available to shareholders holding a minimum of 100 shares of Carnival Corporation or Carnival plc. Employees, travel agents cruising at travel agent rates, tour

conductors or anyone cruising on a reduced-rate or complimentary basis are excluded from this offer. This benefit is not transferable, cannot be exchanged for cash and, cannot be

used for casino credits/charges and gratuities charged to your onboard account. Only one onboard credit per shareholder-occupied stateroom. Reservations must be made by

February 29, 2016.

Please provide by fax or by mail your name, reservation number, ship and sailing date, along with proof of ownership of Carnival Corporation or Carnival plc shares (for example,

photocopy of shareholder proxy card, shares certificate, a dividend tax voucher or a current brokerage or nominee statement with your brokerage account number blacked out) to

your travel agent or to the cruise line you have selected below.

Just google it.

 

You fill it out and send it to Princess, not your TA.

 

You can do it either way. I prefer to have my TA handle it for me. I do not have a fax machine and Princess will not accept the form by email. I fill out the form, scan it to email, along with the proof of ownership, and send it to the TA who then deals with Princess.

As others have said, you do not need the form. But if you prefer a form, Astro Flyer posted it on February 13 at http://boards.cruisecritic.com/showpost.php?p=49080670&postcount=30.

Princess created the form because they receive too many incomplete submissions which slows down processing all of our requests by having to follow up with passengers.

 

Sometimes they receive requests that lack even basic info such as the passenger's info so they're unable to contact the passenger to get all of the necessary info to process their request. The form also provides answers to the application process which is regularly asked about on CC but it's not required to use the form.

You're lucky. I faxed both my sister's and my requests on February 16th and they're not posted yet. Think I'll make copies and mail them in 'cause that's way too long. Wish there was an alternate to faxing or mailing. It would be so much easier to upload to a secure site.

That's really odd. You faxed to 305-406-6102?

Have you called them? I submitted our's on 2/1 and it was issued on 2/4.

I followed the directions on the Carnival page. I faxed a copy of my latest statement showing the Carnival shares and a cover letter showing my name, cruise date, cruise number, and whatever other information they requested. Didn't use any form.

 

Both times I did this the OBC was credited to me within a few hours.
