Refund requirements prior to boarding after testing positive for covid. What form is acceptable?

We are scheduled to leave on January 21st on the Equinox. It appears my husband may have come down with covid as of today, which is within 14 days of sailing. According to Celebrity policy we can request a full refund rather than FCC.  We must provide a form as proof which is acceptable to Celebrity to qualify for the refund. They don't state exactly what this acceptable proof is.  Has anyone had any experience with this and know what qualifies as an acceptable proof? Thanks for any information anyone may have.

Has he been tested other than a OTC home test?  I'm sure they require a copy of the test results.  Calling Celebrity should get the information on what & where to submit the info for a refund.

If you booked with a TA they should be able to provide that information.

Perhaps the paperwork showing that he tested positive?

Check out this link.  I would assume that any officially monitored PCR positive test would be acceptable.  Likely an antigen test too since they accept that for boarding.  But I'd verify with a PCR if you haven't done one already,
