My attention has been directed to finding a house in Mexico for retirement. Once that is out of the way, I can start looking at cruises again. I am a retired family physician who worked extensively in "Vaccine-Preventable Diseases," "Adult Immunizations," and "Travel Immunizations," including some direct work WITH the Immunization Division of the CDC. The most reliable source of Immunization Recommendations for Travel is from the Centers for Disease Control. They make note of each country's travel entrance REQUIREMENTS, as well as RECOMMENDATIONS for U.S. citizens travelling to each country. As ALL immunizations have effects / side-effects, and indications / contraindications, a MEDICAL HEALTH CARE PROVIDER should make the recommendations for YOU. This should NOT be done by a Travel Agent or Tour Provider (IMHO), because they know NOTHING of your own personal health history. I've been to Brazil, and had to have a Yellow Fever Vaccine BECAUSE my itinerary included Manaus, on the Amazon. The CDC website mentioned also notes that, for Brazil: Coastal cities, including Rio de Janeiro, Sao Paulo, Salvador, Recife, and Fortaleza, are NOT within the endemic zone. Vaccination IS recommended for travelers visiting Iguaçu Falls. So, for Brazil, since the Mariner will probably be visiting only coastal towns (possibly Recife, Salvador, Rio, Sao Paolo), a Yellow Fever Vaccine is not needed. If you take an excursion to Iguazu Falls (which is very likely to be offered, probably as a flight from Rio and/or Santos/Sao Paolo), then you WOULD need a Yellow Fever Vaccine. The area of Travel Immunizations is complex. It depends on where you are going, what you will be doing when you are there, whether you will eat the local food or have exposure to certain people or animals, and etc. etc. It also depends on what your own medical history is (do you have an immune-compromised disease? are you on chronic steroid treatment?), what your past immunization history is, what allergies you have, what prescription medications you are on, etc. etc. On top of all of this, local conditions where you are going may change - there may be an outbreak of something at the time you will be going, so the recommendations / requirements may CHANGE. (Remember when an outbreak of SARS kept folks from travelling to Toronto?) My recommendation is that you seek the advise of your physician. If your physician is not well-versed in the area of Travel Immunizations, I would recommend that your physician refer you to either a specialist in Infectious Diseases, or to a Travel Medicine Clinic.
