I will post my usual warning/caution. Read any insurance policy BEFORE you purchase and pay particular attention to the coverage limits. If you have decent health insurance which covers you outside your home country, than extra medical coverage is less of an issue. But if you do not have coverage outside your country, the policies sold by nearly all cruise lines have woefully INADEQUATE medical coverage. Many of those policies have only $10,000 ( or less) of medical coverage...in an era where one can quickly spend over $50,000 for a major medical problem.

The cruise line policies generally stress (and market) trip cancellation benefits more then their medical/evac benefits. Always remember that your cancellation liability is capped at what you actually pay for a trip (which means you can afford it). But your medical liability is virtually unlimited. In fact, in the USA the leading cause of personal bankruptcy are excessive medical bills.

These sites help you understand comparisons of travel insurance & each companies ratings:

http://www.travelinsurance.com

http://www.insuremytrip.com

http://travel-insurance-review.toptenreviews.com/

The cruise line insurance is usually less expensive for the older age sets 70+.

Its better to go direct with the insurance provider because the coverages are usually richer, more policy options (could be overwhelming though), and less expensive for more value. Also, if you have a claim you are going to need to go direct to the insurance company. RCI is not going to help. We used http://www.csatravelprotection.com because of their A+ rating on BBB and strong record on claims payouts. CSA is lesser know so the customer service is better.
