They didn't used to be.  Crisis Centers formed in response to the rise in 1970s drug use morphed into telephone call-in  staffed solely by trained volunteers who answered in their homes when calls were forwarded by a switchboard.  When people in crisis called and the person on the other end lived in their own community, a connection could be made.  Since the Crisis Line were advertised as "Any problem, any time", people would call (anonymously-Caller ID wasn't a thing) before they reached the breaking point.
