Does HAL have a fit to sail form?

Not sure about the specific answer on that one but an email I received a few weeks ago indicated we would be getting a survey we would need to complete prior to leaving the hotel or other place for the port. The context indicated it would be the morning of boarding. Maybe someone can clarify that one.

My cruise isn't for another 3 weeks and I do not expect to be receiving the survey to report back on that one: I just cancelled. 😞

That is cancellation #2 in my short cruising career.

Just reading Celebrity and NCL's fit to sail form. Does HAL have the form yet, or will they?  Celebrity's is inclusive of most diseases that most people over 70 have, including cancer.

Not as of this moment, 3:50/pm 3/13/2020. I burned off that NCL form from Internet and took it to my Dr., yesterday, who filled it out after a short physical. It appeared to be a 'Boiler Plate' form akin to what they ask pregnant gals etc  to submit. I sail Sunday.
