Bermuda pre-arrival form?

Booked on an October Adventure of the Seas from NJ - Bermuda.

Stumbled across a form and $40 fee to enter Bermuda.
Reviewing our booking there is no mention that we need to fill out an extra form to enter Bermuda.
We have no problem with the form or the extra $40 to get there if we have to.

Our question is do we have to do it for the cruise or is it something that the RC takes care of for passengers in the background like what they do at other ports?

You have to complete the Travel Authorization application yourself. Each person over the age of two has to apply and pay the $40 fee.  It's a COVID-19 related requirement, not at all similar to entry requirements almost anywhere else with  the exception of Canada, but Canada does not charge a fee for their somewhat similar ArriveCAN application process.

That is your loss. Bermuda is an island paradise and hope to go amy more times. The island does have quirky politics and culture but that is part of the charm.
