Where is the best location on a Transatlantic cabin to Rome

POSH - Port Out (from Europe to North America), Starboard Home (from NA to Europe).  If you want the best chance to catch some sun on a balcony cabin get one on the starboard (right) side of the ship.  If you’re concerned about motion sickness best to get something low and near the centre (midships) of the ship.

Does the same apply to a northern hemisphere westbound transatlantic in August? Would like morning sun and afternoon shade on balcony. Port or starboard?

POSH applies to Northern Hemisphere, Atlantic Ocean. Port gets you more sunny conditions going "out" to North America. To achieve what you seek, you would want an AFT balcony. If Aft is not available or desirable, something Portside will give you PM shade during the portion of your transit where you are headed more south than due west.

 

Morning sun/afternoon shade in a cabin alongside the length of the ship would only be attainable if going north-south or south-north!

I don't think it will really matter...Choose the cabin and location that will work best for you based on your needs on the ship and close to the venues you'll be using...Not on what you "might" be able to see or where you "think" the sun might or might not shine

On a TA to Rome, or anywhere in the Mediterranean, we would choose the port side. Sailing east, puts the shoreline off the port side so for us, that means more scenery. It would also mean less full sun on the balcony, so consider that if you like to sit in the sun.
