You can pre-request a cake and there are other items you can purchase. Make arrangements weeks prior to the cruise. This would be similar to other types of celebrations, such as anniversaries, which aren't noted on cruise reservations or supporting documents (or not usually!). You can call the cruise line in advance to request something (especially if it's early in the cruise), or you can make some request once on board. We've done it both ways. Quietly talk to whoever seems most competent in the DR a few days before and tell them you're celebrating and what you would like done/what they can do.
