Can someone please add a link so I can download the shareholder obc form.

 

Cannot find it on Carnival website or using search function here.

form is not needed. just fax them your information like name, booking number, sail date, ship

Princess has a form, but it seems Carnival does not.

No form. Just compose a short letter asking for the OBC, giving your name, ship, sailing date, and booking number, and enclose a statement from your broker, or whatever showing at least the 100 shares of CCL stock. Fax, e-mail, or snail mail that to Carnival Guest Services, and you should receive a short note from Carnival that they have received the request, and are posting the OBC.

When you send your statement showing shares owned, I'd suggest blocking out any other information. They only need my name, broker and the one line that shows your Carnival holding.

I just did it by email and im not very tech, so didnt know how to block stuff out. No where on any online page does it say anything but Hello (first name) no where was my last name, they said not sufficient, so i had to ss the docs for monthly from adobe to get it.

I print out the brokerage statement I want to send to Carnival. Then, I take a black magic marker to it, and it looks like a DOJ document that has been redacted before being released. Then I scan it in and send it off via e-mail to guestadmin@carnival.com

I black out my other holdings and account number.

Can be found on Carnival website, investor relations section. Will provide a address where to send information.
