Prepaid gratuities cover your stateroom steward, dining staff, and other behind the scenes people. If you purchase a drinks package or specialty dining package, you will be charged additional gratuity for those packages.

I did this 12 years ago, so my information might be dated. I did this on the Crown. The Captain has a prepared thing, but I'm sure if you wanted to do your own thing it wouldn't be an issue. The ceremony took place in the chapel. We could not pick our day/time. It's based on the Captain's availability. Ours was scheduled for the first formal night right before the main festivities.

The process at Cape Liberty is you drop off luggage then park. Usually the porters are right there to get your luggage. Not sure if Star Class makes a difference on that.

Doubt it. The restaurants are only included on Mardi Gras (and soon to be Celebration) because they are an extension of YTD, so there is enough space for everyone to eat dinner. The other ships can fully accommodate everyone in the MDR.
